In persons dependent on prescription opioids, tapering with buprenorphine during a 9-month period, whether initially or after a period of substantial improvement, led to nearly universal relapse in the National Drug Abuse Treatment Clinical Trials Network Prescription Opioid Addiction Treatment Study, presented here at the American Psychiatric Association 2010 Annual Meeting.

"There has been virtually no research on the treatment of persons dependent on prescription opioids, in spite of the major increase in prescription opioid abuse and in the numbers of persons entering treatment for addiction to prescription opioids," said Roger D. Weiss, MD, professor of psychiatry at Harvard Medical School, Boston, and chief of the Division of Alcohol and Drug Abuse, McLean Hospital, Belmont, Massachusetts.

The study, which is the largest treatment study ever conducted for prescription opioid dependence (POD), sought to answer several questions regarding the optimal length of pharmacotherapy, the value of intense counseling, and the role of chronic pain.

Specifically, the study asked whether adding individual drug counseling to buprenorphine-naloxone (a semisynthetic opioid and a partial agonist) plus standard medical management improves outcomes, what duration of buprenorphine is best for these patients, and whether presence or absence of current chronic pain influences outcomes.

"The trial was designed to help the physician manage patients who are dependent on opioids and want off the drugs but refuse treatment in a drug abuse treatment program," Dr. Weiss said.

The study enrolled 653 persons with POD and offered them standard medical management, which included buprenorphine (usually 12 - 16 mg maximum, adjusted for addiction, not pain), an initial 1-hour visit, and weekly 20-minute sessions with a physician who counseled the patients and monitored for drug adverse effects. Half the group remained in this standard medical management (SMM) group and half received enhanced medical management (EMM), which included twice-weekly 60-minute individualized drug counseling focusing on interpersonal issues, coping with triggers and high-risk situations, homework, and so forth.

Under a somewhat complicated schema, patients were evaluated after periods of individualized buprenorphine tapering and maintenance and were assessed for abstinence from opioids at various times.

Study Population

All patients had a Diagnostic and Statistical Manual of Mental Disorders, 4th Edition, diagnosis of opioid dependence and had used opioids for at least 20 of the last 30 days. Other substance abuse disorders were allowed, with the exception of active heroin use or history of injecting heroin. All patients expressed an interest in stopping opioids.

The population was made up of 60% men, was 91% white, had a mean age of 33 years, was likely to be cigarette smokers (71%), and had been using opioids for an average of 4.5 years. Most patients had received some college education and were employed full-time. There were no significant differences at baseline between the SMM and EMM groups.

A number of patients reported current chronic pain (42%), and some were taking opioids for this condition. Although current polypharmacy was uncommon, many patients reported a lifetime history of heroin use (23%), alcohol abuse (60%) or dependence (27%), cannabis abuse (47%) or dependence (15%), and cocaine abuse (32%) or dependence (18%). Stimulant and sedative use were less common.

Opioids used within 30 days included sustained-release oxycodone (35%), hydrocodone (32%), immediate-release oxycodone (19%), methadone (6%), and others (8%).

Thirty percent of subjects had received some previous treatment for opioid dependency, primarily "self help" (59%), inpatient/residential treatment (42%), outpatient counseling (40%), and methadone maintenance (31%).

"For most subjects, this was the first treatment for opioid dependence," said Dr. Weiss.

Treatment and Maintenance

Treatment success was defined as 4 or fewer days of opioid use per month, no positive urine screens for opioids for 2 consecutive weeks, no other formal substance abuse treatment, and no injection of opioids.

Phase 1 included 1 month of tapering and 2 months of stabilization. At the end of this time, few patients were successfully treated, and enhanced management did not influence the results, Dr. Weiss reported.

In the SMM group, only 7% met the criteria for success, as did just 6% of the EMM group (P = .45). "Nearly all patients relapsed after a 4-week taper," Dr. Weiss said.

Patients who relapsed were asked to enter phase 2, at which time 360 patients were again randomly assigned to SMM or EMM and received 3 months of buprenorphine stabilization, then had treatment tapered for 1 month, with a 2-month follow-up.

At the end of the stabilization (at week 12), substantial improvement was noted for 52% of the EMM group and 47% of the SMM group, though again there was no additional benefit to enhanced management (P = .3). Substantial improvement was defined as abstinence for 3 or more of the final 4 weeks of buprenorphine stabilization (urine-confirmed self-report).

However, by the end of the stabilization period, many patients had relapsed again, Dr. Weiss reported.

"We went from an average success rate of 49% to 26% at week 16,"he said. At week 24 (8 weeks posttaper), only 9% of patients remained successfully treated.

"At the end of the study, we were back into phase 1 territory," he said. "Seven of 8 patients doing well on buprenorphine maintenance had relapsed."

Predictors of Outcome

The only predictor outcome was ever-use of heroin. At week 12, improvement was noted for 37% of those reporting lifetime heroin use compared with 54% of those without such a history (P = .003); at week 24, this was 5% and 10%, respectively (P = .13). "Having dabbled in heroin was a bad prognostic sign," Dr. Weiss observed.

The presence of chronic pain did not influence outcomes. Patients with chronic pain were equally likely to enter phase 2 (indicating early treatment failure) and were equally likely to be substantially improved at week 12 of phase 2 (53% vs 47% for those without chronic pain).

Chronic pain tended to be lumbar/sacral (65%) and classified as only moderate (median 4.4 on 10-point scale) but was of long duration, as more than half the patients had suffered from it for at least 4 years, he said.

"Interestingly, we found that in many cases the patient's pain got better," he added. More than half the subjects reported at least a moderate reduction of pain from baseline (≥30%), and one third had a substantial improvement (≥50%).

Nevertheless, Dr. Weiss said one cannot assume that buprenorphine itself improved the pain, as there was no control group, "but it is an intriguing possibility," he commented.

Sean Mackey, MD, PhD, associate professor of anesthesia and chief of the Division of Pain Management at Stanford University, Palo Alto, California, who delivered an overview of the treatment of pain in patients with addiction at the session, commented on the current study for Medscape Psychiatry.

He was particularly interested in the finding that persons with a history of heroin use had worse outcomes. "Could it be that prior exposure to heroin fundamentally alters the neurobiology in this group such that they need higher doses of buprenorphine to prevent relapse?" he asked.

Dr. Mackey maintained that the study is important because it asked a clinically relevant question: "Does putting people on a short period of buprenorphine maintenance combined with counseling lead to reductions in relapse? It's a great idea, and a wonderful hypothesis, because if it does work then this would be a huge win. We would not have to use extended maintenance. Unfortunately, it did not work, but the study needed to be done."

He further noted that the standard management group was likely getting better care in this study than is delivered in usual practice, which may have diluted potential differences.

Dr. Weiss has reported receiving research support from Eli Lilly. Dr. Mackey has disclosed no relevant financial relationships.

She ran many of her sites independently for several years after leaving Narconon Arrowhead staff) and still owns them as far as I know even though she is staff at Narconon Eastern United States in Clearwater.  She is probably now flowing victims from her sites to Narconons on the East Coast instead of Arrowhead in OKLA.  She was Director of Promotion for Narconon Eastern United States ands has recently been promoted to Deputy Exec Director for Expansion of Narconon. Some of her sites are Michigan related, as she feeds people to Narconon Freedom Center there, since joining staff at NN EUS. Her area would cover NNs in MA, FL, MI, GA TX, LA, MS, CT and any other that might exist in the eastern area of USA. More info:

"George" called this phone number 1-866-597-9931 from that website and spoke to a woman who put him right through to "Mike" ( the infamous Narconon reg Mike DePalma ) who actually states that he works for Narconon Fresh Start but refers to other places, too. He then tries selling Narconon at facilities Fresh Start owns ( CA, CO, TX, NV) but "George" is from Oklahoma and Radio Paul tells us "George" mentions that he sees that there is a Narconon in OK but "Mike" says it's better to have "George's" brother Jim go out of state. Fact is, Narconon Fresh Start didn't want to send him to a facility they don't own.

Radio Paul's show at Radio Fairfax tonight plays a  segment of this recorded call during an interview with David Lee of Intervention Services, Inc.

The fact that he even mentioned Narconon Fresh start right off hand is somewhat surprising, but then again, "George" told the girl receptionist that his family had money.  It probably would have been a completely different conversation if he said otherwise.  It's all about the GI (Gross Income)$$$. 

You're right about him only mentioning the Narconon Fresh Start facilities and leaving out Arrowhead in Oklahoma - or the other centers' locations.  I could tell that was one of Larry Trahant's websites the second I looked at it. There are hundreds of them.

I know what you mean! Yes, having a caller on line with money probably tripped Mike DePalma up on that, lol.

Funny how Mike  got on the line in like 2 seconds, not enough time to have chatted with the woman who "George" spoke to, yet he knew how many years "Jim" was using. He was listening in the whole time, of course. 

What surprised me the most was how surprisingly stupid he sounded explaining the program, like he either didn't want to bother. If I didn't know him better, it would sound like he just didn't know how to answer the question well. Hope we get to hear the whole tape.

By the way, an unnamed birdie told me it was you who originally mentioned that Larry Trahant was running a company called Intervention Specialists. Thanks for helping start the ball rolling on checking out that fake referral site :)

Googling the phone number gets a huge list of sites for every state and multiple cities in most states. Seems to be the normal MO for this kind of web saturation....then I find that NN Riverbend uses the same phone number: http://www.roadtorehab.com/Drug-Rehab---Narconon-Riverbend-49428.html

The cloaking can fool people looking for rehab and some of it seems set to prime  a searcher for rejecting other forms of rehab in favor of the NN styled scam. The cloaking is not effective if you have been hanging around tipping point and getting some education.... ;D

There are blog categories for communication skills, life skills, getting your freedom back and happiness. The descriptions are very telling. Also, in the descriptions of what to avoid, they discourage any treatment that uses a disease model for addiction.

It is definitely a site owned by Narconon Riverbend in Louisiana.  It used to be Larry Trahant's site from NN So Cal/Fresh Start.  He is from Louisiana originally and then gave it to them.  They paid Dan Manson at NN Vista Bay 40K to rebuild it and drugabusesolution . com for them as well.  Dan has done side jobs for extra Bridge money (i.e. to give to the church) as has Derry Hallmark at Arrowhead, and others.  Derry made an extra 100K building and selling sites to FSMs while he was a staff member.

which can search a site's ownership or contact name, a contact phone number and other things, and give you a list of sites using that contact name or contact phone number.  Found a bunch owned by Pamela Anderson, registered by some girl in NC, which need to be sorted out  - fake referrals  from AFR sites. 

But the site is very useful. I searched using Derry Hallmark's name and this is what came up

The whois service queries the whois databases managed by domain registrars and displays relevant information about the owner of a domain name.

Myself and other members have a sh*t load of fake referral links that have to be added to this thread. I also have pdf copies of site domain ownership registrations before most began using proxies to hide that info. That's another project to be worked on after more important ones get finished

 Is this information being put together and crafted into something to be sent to a governing agency? Or is this just for informational research purposes?

 I would gladly step up and do a cover snail mail and online email letter to both state and federal agencies that would look into this.

 I would need rock solid fact based information as to what if any abuse of or laws broken that would bolster the complaints merits. Then adding the research material that supports those claims!

I have been collecting information and pdf copies of these sites for years. I try to help victims who have been ripped off by them. One of the difficult parts of this task is getting people to recall where they first made contact with Narconon. Many do not keep a record of the website they found, or the specific key words they searched for. Once they contact the site either through the phone number or through the online 'contact us' page', they speak to someone who then transfers the call or gives them a name and number to contact. By the time they have been ripped off ( which could be days, weeks or  months later) I am lucky if they are able to find the toll-free phone number they contacted. Unfortunately., most of these numbers are embedded on the sites so you can't find them through a search by Google.  So finding the website is important but usually not fruitful. In the cases where I was able to get the site from the victim, or locate it based upon key worlds they searched that lead to it, complaints have been made, or lawsuits filed that contain the information supporting the fraudulent activity. This information includes the site whois information, the names of people involved in the line of sales and connecting them with Narconon and or Scientology whenever possible.

After they filed the Affidavit, before this case went any further, Narconon ran to settle, knowing that the issues of the fake sites and phone coordination with NN So Cal would be open to litigation. The Locatelli's, who needed the money to pay off their growing credit card debt and be able to pay for another program, decided to settle. I could not blame them. It was a long hellish process for them. Sarah Locatelli filed this on her own and they did not have the resources for an attorney for a long drawn out lawsuit. They just wanted their money back and got it.

Now in the following complaint, the fraud was even more documented and lead to the complainant getting a refund.

This complaint has been on the web since July 2009 and has been read by 8,765 people.  Hopefully many of those who read it were prevented from being scammed like the author was.

So I continued collecting site information as I came across it, but did not always have time to dig deeper into the whois info ( especially if it was proxied, as many began to be), taking snap shots or printing PDFs of what the site said, or the whois said, about the site and sometimes posting it when I had a chance. I also collecting it as well with the additional hopes of documenting the telecommunications and telephone fraud to the FTC . As well, ethercat, mefree and myself have been collecting and sharing information about such sites over the past several years, in the hopes of one day having a data base of them made so that people could know what what sites to avoid, fake or actual. These sites are usually found as we go along doing other tasks on the internet. Because it's such a big task to collect & gather them all, it's and because we are all swamped with other priorities, I just started this thread to make it easier to post them as we come across them, but these are by no means all the sites we have come across. 

The bottom line is that this Fake Referral is a business activity and appears to be an organized RICO activity and one can't just go to the government with a few complaints and no specific proof connecting the RICO activity. One has to connect all the dots and the way these are set up, it's very difficult to do on a multiple site basis but we figure eventually there wil be enough information to help make a case and get some real action.

Keep in mind that the govt, the FTC, does not take these cases on an individual basis. They need lots of complaints about a specific activity or website, and only then do they sue the parties for fraud but not monies to benefit those ripped off. So I'm looking at the RICO. And looking at seeing all the info up at some point on the net but it is a big task. Registrations change owners, change availability of owner info by use of proxy's. One usually has to pay to get the info, a costly thing. And subpoena to get a proxied owner's name.

That is not to say that I don't encourage people to file FTC complaints, because I do. And Better Business Bureau Complaints, and licensing agency complaints, and state consumer affairs complaints.... but these complaints have to be very specific or else they get tossed.

I believe in the near future all this information will come handy if and when a class action lawsuit is filed by willing attorneys on behalf of students and those who paid for their program, each having the common element of having been defrauded by a fake referral website that leads them to one of a number of known  Narconon sales reg's. Don't think I haven't raised this idea with attorneys over the years. It's just that it's a humongous task to get people to recall where they first went when they were then referred to Narconon or fake names similar to programs they asked about ( referred to "Joshua Hills" instead of "St Jude's")

 Thank you Mary for the explanation. on that note,would it be advantageous to have this type of thread as a central point of both focus and placement on this or another website?

 The false or misleading Narconon referral websites are it seems the main entry point with which the consumer first becomes exposed unbeknowst to themselves both Narconon and Scientology.

 If in fact a website is already up concerning this information,I feel that site should be nurtured and positioned so the general public can review the material and links. This material you and others have researched reveals the cornerstone which the Narconon/Scientology scam perpetuates itself.

The fact that he even mentioned Narconon Fresh start right off hand is somewhat surprising, but then again, "George" told the girl receptionist that his family had money.  It probably would have been a completely different conversation if he said otherwise.  It's all about the GI (Gross Income)$$$. 

You're right about him only mentioning the Narconon Fresh Start facilities and leaving out Arrowhead in Oklahoma - or the other centers' locations.  I could tell that was one of Larry Trahant's websites the second I looked at it. There are hundreds of them.

Summary: I called the 866 # on www.interventionspecialists.com I spoke with Ryan W. who promised advice and help. I was duped into a sales pitch for NarcoNon.

Resolution Sought: First of all, I would like a sincere apology for NarcoNon's deceiving business practices. Second, I strongly advise Ryan W., Mike, and all representatives of NarcoNon to cease and desist attempts at communication with me, xxxxxxxxxxxxx. xxxxxxxxxxxxxxxx. xxxxxxxxxxxx, xxxxxxxxxxx, xxxxxxxxxxx. Do not call any of us. Do not email any of us. Do not send us anything in the mail. Delete all of our contact information from your files and do not attempt to contact any of us ever again.

Detail: I found this website: www.interventionspecialists.com through a Google search in trying to find an interventionist to help with a family situation dealing with addiction. The website specifically states, "Fill out the form below and a counselor will get back with you to help you locate an interventionist and/or treatment program." They also have a toll free #, which I called (1-866-597-9931) I was transferred to Ryan Warczak who told me he was a "counselor." I described the situation and he promised to set up a conference call with me, my wife, brother-in-law, and father-in-law & mother-in-law to provide "guidance" on how to proceed with an intervention, including why we should hire a professional interventionist. I spoke with Ryan the next morning (1/29/13) to arrange the conference call, he said he was busy but that he would have another counselor handle the call. The other counselor, Mike, began the conference call with me and my in-laws by asking very personal information for about 45 min, and then began a sales pitch for NarcoNon Fresh Start. It was easy to discern over the phone that Mike was reading from a script. When we stated that we did not want a sales pitch for a treatment center, but rather the "guidance and advice" that we had been promised and a list of potential professional interventionists to contact, Mike got defensive and then said he would follow-up with a list of contacts. He did not provide those contacts. They continue to call and badger us daily.

Yes, registrars *should* reset the domain creation date when it changes ownership through expiry then scavenging, but most don't these days.

My Narconon list is only a partial list, naturally. Several years ago, they were multiplying so fast that I would have been swamped too tracking them all, especially with the FSM sites. It does burn me that people are coming forward now who searched back then but didn't see the critical info. No more.

Per Wickstrom is especially slimy. His sites don't directly include his toll-free contact number, but use javascript to alter the page displayed with data from the mothership. That's easy to search for, and I'll post about that soon.

List updated with David Holtz' other domains. He was a very naughty boy, and used the Fort Harrison for the registration address of some of his sites. He even used it on some of the sites themselves.

It might be good to add narcononreviews.com, since Narconon owns that one too.  I have narcononreviews.net and .org, so please don't add them.  The .org redirects to the .net.  They also snatched up a bunch of other "reviews" URLs when they became aware of narcononreviews.net (which was actually before it opened to the public). 

Another suggestion I have, if you can have your script add it without too much work, is to add a 'rel="nofollow"' tag to your links to the Narconon sites so you don't give them any link love and inadvertently boost their search engine rankings: Google Webmaster Tools - Use rel="nofollow" for specific links (https://support.google.com/webmasters/answer/96569?hl=en).  It might also improve your site's ranking because it won't look like another "add your link" type site, which look spammy to the googlebot and get downranked.

I probably don't need to tell you this, but additions by the public should be moderated, lest Narconon starts adding our sites and their competitors' sites.

Another suggestion I have, if you can have your script add it without too much work, is to add a 'rel="nofollow"' tag to your links to the Narconon sites so you don't give them any link love and inadvertently boost their search engine rankings: Google Webmaster Tools - Use rel="nofollow" for specific links (https://support.google.com/webmasters/answer/96569?hl=en).  It might also improve your site's ranking because it won't look like another "add your link" type site, which look spammy to the googlebot and get downranked.
The problem is that nofollow is a global setting with Mediawiki, and I had to trade off between giving all the news articles a bump, and CoS's sites. When I get the chance, I'll add a page setting for that. There's enough content on the site that I don't think it's hurting ranking too much, and when I add RDFa Lite semantic markup, it'll make up for any downrank in a big way.

Now that I have a site that I directly control, I'll be adding a way for people to submit new entries.

I probably don't need to tell you this, but additions by the public should be moderated, lest Narconon starts adding our sites and their competitors' sites.
Changes will be filtered through me first from known contributers. There were wikibots in the first week trying to add junk articles with spam links, and I don't have Wikipedia's human wave to delete those.

Meanwhile, I'm trying out different setups on the other Pi.

I'm looking at a phone number with a legion of sites attached, trying to decide if it's Narconon or not.

The site name formats are drugrehab<town name>.com. The tricky part is that the sites do list local non-Narconon rehabs as well. Short of calling their number and using my terrible social engineering skills to see if they only steer people to Narconons, it's hard to call it. It smells like a Narconon referral setup, and they've spread such a wide net of sites, all registered in 2008, that they can afford to let a few fish go to other rehabs.

Narconon Southern California did similar with drugrehab<state>.com in 2003.

Note: Toll free numbers will report your phone number to them, independent of any caller-id blocking that you use. (It's their dime for the call, so they get the info.)

Combining the phone number with some other search terms got a few appearances of the number that weren't part of the cookie cutter sites.

  • JLB Ranch in Warner Springs, California
  • Sunshine Summit Lodge in Warner Springs, California (which has also used the names Narconon Warner Springs and Narconon San Diego)
  • Narconon Fort Collins in Fort Collins, Colorado (which has also used the names A Life Worth Living, A Life Worth Saving, Narconon Colorado, and Fort Collins New Life Center)
  • Narconon Lone Star Victory Ranch in Harlingen, Texas
  • Rainbow Canyon Retreat in Caliente, Nevada (which has also used the names Narconon Nevada, Rainbow Canyon Recovery, and Rainbow Ranch)

and two Outreach Centers, which I believe are Drug Education centers, not rehabs:

  • 866-407-4380 (sites with states in the name such as drugrehabmontana.com, drugrehablouisiana.com, and others)
  • 866-395-1680 (used for usnodrugs.com pay per click ads, also listed on other sites, see above project ideas)

Often, you can search the numbers to find other domains that use the number, but sometimes the numbers are shown in images instead of text, and therefore are not listed in the search engines.  I believe this may be to keep it from being apparent how many sites that referrer has, but not sure.  Maybe one of our resident experts can chime in here on why the images are used.

I'd give that 866-567-4793 number and those domains about a 1 in a million chance of being something other than Narconon.   ;)

Yep, it sounds like I should just grab everything in a net, tag them "99% Narconon", then sort out what I've caught later. (And hope that they goofed on some when hiding their tracks.) Maybe a Google Map with pins for each town would show a pattern?

It's too bad that there's no toll-free number registry like WhoIs, to see when they picked up that number and dropped the other one.

The Google result, boiled down resulted in 166 domains, yeesh! Some of the sites seemed confused about which state they were in, with rehab places listed near the town of the same name in different states.

They probably took the government's list of rehab places and munched it up.
