I am confused.  And I'm quite sure that's the intention.  Because if I'm confused, imagine how a parent searching for a rehab feels.   :-\

What I find interesting for the 163 North Ave address, what the IRS shows is this as a tax exempt facility:

EIN: 46-1346441
Legal Name: MY BEST DRUG REHABILITATION INC
Address :  163 NORTH AVENUE BATTLE CREEK, MI   49017

And the initial 990EZ for 2012, found at: http://990s.foundationcenter.org/990_pdf_archive/461/461346441/461346441_201212_990EZ.pdf shows they are telling the IRS this facility is to, "Train employees to care for detox patients." Not a word about actually being a detox facility, and no mention of "Purr" Wickstrom either.

What's even weirder is Tranquility Detox shows up in the IRS Select Check search as being in Richmond, MI, but the 2012 Form 990 at http://990s.foundationcenter.org/990_pdf_archive/452/452137035/452137035_201212_990.pdf shows the same 163 North Ave, Battle Creek address as My Best Drug Rehab.

Something is rotten in Michigan, and it's not the fish.

This has been posted about before. Tranquility Detox is a fake name for a detox facility at that address owned by 2 of Per Wickstrom's drug facilities:
Best Drug Rehabilitation and A Forever Recovery. The licensing for the location is under both companies now. At one time they were both using same one space facility for detox at that location but since they got complaints, the state made them create 2 separate detox suites at that location,  one for each company, covering each license. Tranquility Detox, LLC is a separate entity, as are Best Drug Rehabilitation and A Forever Recovery, all registered in NV now I believe, doing business as aforeign entities in MI.  Members here and Miss Fortune have reported on all this in the past

Are you sure you don't mean Richland, not Richmond?

The original corporate Resident Agent's address is in on Westwood Dr Richland, MI . Stephen Anderson, Per's son in law. 

The other address is a business location.

Stephen Anderson is no longer with them.

My bad. I misread the 'Select Check' entry. It is Richland, MI for EIN 45-2137035 Tranquility Detox, not Richmond.

What bugs me is the business address on the Form 990's is supposed to match the 1023, and doesn't. If they changed addresses they are supposed to update with the IRS, but based on what's in the 'Select Check' search they apparently haven't.

The 1023 form is filed in one year, first year. It is a permanent record of original tax exempt application process. They get a letter when the 1023 process is finished. Approved or not, they get a letter stating so.

The 990s are subsequent annual income tax form report filings, with address listed at current location, updated as needed on that form as goes on. Not all entities remain at the same address.

When you say 1023 form, are you referring to the Q&A the IRS sends an applicant? Or are you referring to the Non-Profit Tax Exempt list the IRS has on line?

Do you have a copy of Tranquility's 1023 form? Would love to see it

Unfortunately I don't have an actual copy of the 1023 application for exempt status. I'm basing this on what I was told by a lady at the IRS when I first started digging into NN/Cof$ Form 990's. What she told me is that when a 1023 is approved the information, including the business address on the 1023, is put into a list of Exempt Organizations. Otherwise known as Pub 78, which the IRS has been turning into an on-line database searchable through their 'Select Check' exempt organization utility.

The business address used on the 1023 is supposed to be what's used on the Form 990. If the address changes after the initial approval, the IRS is supposed to be notified by checking the "Address Change" box on the Form 990 so the Pub 78 information, not the original 1023, can be updated with the current address.

It's pretty obvious Tranquility Detox did not do that, because their 990's show the Battle Creek address, but the Richland address is what's still in the EO data listing. I just pulled this out this morning:

Tranquility could have called in the change of address or filed a Form 8822-B form changing the address.

I don't see anything really nefarious because I know for a fact that  Stephen Anderson left a while back, that things are changing for Tranquility... they are trying to get Indiana and in the Grand Rapids area Wright Twp facilities they bough but are not yet licensed for   -- so and if they failed to note that a change occurred by IRS change of address form, call or noting it on the last 990, yet used the  most recent address on record with the state dept at the time, it's no big deal. The Grand Rapids address is pretty new - they are not even allowed to open their facility there yet -- the zoning use still pending in court on appeal -- and extentioned 2013 returns are not due until after October 15th ... so may not see the updated address until they file that one.
Remember, business office address is different from business address and as long as they file with the state of a updated resident agent address in which to be process served, age get the business address to the IRS at some point in the year by call, form or on a 990, the IRS does not make a big deal of it.

I suspect that Per Wickstrom deliberately engineered the confusion between "Tranquility Detox" and "Tranquility Detox, Inc."

Both were incorporated on the same day, probably the same time, with known flunkies of his as agents. Normally, as part of incorporation, a name search would have turned up the similar names and rejected one of them. Because both were done at the same time, the system didn't kick up a fuss right away.

Currently, at the Michigan state site, "Tranquility Detox, Inc." and its assumed name of "Tranquility Detox" (colliding with the other incorporation) have "Status: (CONTACT OUR OFFICE AT 517-241-6470 TO OBTAIN DOCUMENT OR EXPLANATION) Date: 7-26-2011"

Intriguing. Is "Tranquility Detox, Inc." an operating company or not according to Michigan? If not, it renders the Indiana and IRS registrations kind of moot.

Looking at the licensing list, I wonder if Per is claiming the "Service Category Specialty EXPIRATION_DATE" of 12/31/2999 for CAIT and SARF means they can still operate, even though their license expired 07/31/2014??

More confusion.

There's still no license for Tranquility Detox in the online system. Instead, there was one for A Forever Recovery at the 163 North Avenue, Battle Creek location that expired 07/31/2014.
